Move In Liaison

|Description:||The Move-In Liaison is primarily responsible for maintaining the budgeted census in Centerpointe Care Center, Eastside Terrace Short-stay Rehab and Assisted Living by conducting tours and assisting with the move-in process and by collaborating with referral sources as well as the Interdisciplinary Team regarding the move-in process. The Move-In Liaison links the resident and family with the Health Services Team staff to ensure continuity of care.|
The Move-In Liaison participates in planning, facilitating the move in procedure of Centerpointe Care Center, Eastside Terrace Short-stay Rehab and Assisted Living.
The Move-In Liaison participates fully in the Health Services Team, providing leadership and supportive functions to Health Services colleagues, and contributing to the regulatory and fiscal viability of Lakeview Village.
The Move-In Liaison provides leadership in nurturing and mentoring a person-centered culture and resident direction, strengthening the ability of all Health and Home Services to grow as effective operations, and promotes individual and team learning within an environment of Whole-Person Wellness.
|Duties:||•Maintains and develops an average daily census in congruence with what the facility has budgeted.|
•Coordinates the Move-In process. This includes obtaining information on residents prior to move-in, making independent decisions on residents appropriateness for admission, and signing in residents to the neighborhood in which they are moving.
•Builds relationships and collaborates with hospital social workers and discharge planners to ensure referrals and a smooth transition to Lakeview Village.
•Follows up on Lakeview residents who are in the hospital daily until they are discharged from the hospital.
•Coordinates the proper placement at Lakeview by working closely with the Independent Living Social Worker and other team members as appropriate.
•Provides assessments to potential admit to Lakeview Village by going to the hospital to make an independent decision on whether the person is appropriate to move into Lakeview Village.
•Receives referrals, tracks them, and provides appropriate follow-up using the “Red, Yellow, and Green Light” form to determine appropriate move-ins to Medicare and other neighborhoods.
•Ensures that all residents who fall under the “yellow or red” stop light process are reviewed by a nurse prior to acceptance.
•Ensures that all residents moving into the Short-Stay Skilled Rehabilitation are determined to be appropriate and that all needs can be met according to the “Green Light” form and sees that they are moved into the appropriate location on campus without additional approval. The Move-In will work closely with the Neighborhood Coordinators, ADON and/or Central Supply to ensure that the proper equipment and care can be provided.
•In the interest of census building, is effective in conducting resident/family tours of all licensed areas and provides information and requirements regarding the move-in process.
•Responsible for managing and obtaining approval from all Managed Care Insurances prior to move-in.
•Responsible for ensuring that all residents moving to the Eastside Terrace Short-stay Rehab or Centerpointe Care Center under Medicare Part A meet appropriate requirements.
•Update the face sheet in AOD. Educate the clients regarding person centered concept, and what to expect.
•Responsible for the actual move-in of residents and room assignments working closely with the ADON or Neighborhood Coordinator on as needed bases.
Move In Liaison
•9:00-5:30 or 9:30-6:00 PM Tuesday-Saturday with some Sunday Requirements
•Must be a LPN, SW, or have experience as a Move IN Liaison
•Need to have experience as a Move In Liaison for Acute/Sub-Acute setting
•Needs to have great computer skills, organizational skills, marketing skills, and communication skills
