Medical

Two choices for Medical Coverage provided by Blue Cross/Blue Shield of Kansas City:

·         Blue Care Base Plan - Health Maintenance Organization (HMO) - $30 copay for office visits with primary care physician and $60 copay for specialist visits.  Ability to self-refer to specialists in Blue Care Network. $10 copay for routine eye care. 30% coinsurance for inpatient and outpatient services.

·         Preferred Care Blue - Preferred Provider Organization (PPO) Plan - Receive services from any hospital or physician but receive greater benefits when the Preferred Care Blue Network is used.  Deductible and coinsurance vary depending on network or non-network services.

·         Medical plans include a prescription drug benefit; with a $10 copay for Tier 1 drugs, $50 copay for Tier 2 drugs and $70 copay for Tier 3 drugs at participating pharmacy locations. A mail order prescription benefit is also available.

Dental

Dental Coverage provided by Delta Dental:

·         Preferred Provider Organization (PPO) - Reimburses 100% for diagnostic and preventative services, 80% for basic services and 50% for major services.  $25 deductible per individual ($75 family maximum) per calendar year for basic and major services only. $1000 annual benefit maximum.

Vision

Receive discounted prices for frames, lenses, contact lenses if enrolled in the medical insurance at specific participating providers.

Life Insurance

Life insurance in the amount of $15,000 provided at no cost to the employee. Lakeview Village also offers voluntary life insurance and matching accidental death and dismemberment that employees may purchase through payroll deductions. Employees select the amount of extra coverage for themselves, their spouse and/or children.

Supplemental Insurance

Offers employees the opportunity to purchase supplemental insurance products such as a long-term disability plan, a short-term disability plan, a cancer plan, an accident plan, a critical illness plan, and an intensive care plan. 

Cafeteria Plan (Section 125): Premium Savings and Flexible Spending Account for Medical and Dependent Care Expenses

This plan gives employees the opportunity to have health and dental premiums deducted on a pre-tax basis.  It also helps pay out-of-pocket medical and dependent care expenses while increasing your spendable income.  By contributing pretax dollars to a reimbursement account, you lower your taxable income; therefore you pay less in taxes.

**Coverage for the benefits listed above begins the first of the month following 60 days of full time service.  Benefits are available to full-time employees who are scheduled to work 64 hours or more every two weeks.**

403 (b) Retirement Saving Plan

This is a tax deferred retirement plan much like a 401 (k) plan in a for profit organization.  Lakeview Village will match 100% of the first 3% and 50% of the next 2%.  Employees are eligible for the employer matching contributions after one year of service with 1,000 hours or more. A variety of investment options are available.

Holidays

We provide 6 paid holidays per year for full-time employees.  There is no waiting period to be eligible.

Paid-Time Off (PTO)

Full-time and part-time eligible employees will accumulate a specified amount of PTO each pay period based on regular hours worked. PTO can be used after six (6) months of employment.  The amount of PTO accrued will be based on years of service and hours worked. There will be an increase in the PTO accrued for employees with greater than one (1) year, four (4) years, nine (9) years and fourteen (14) years of service.

In the first year of employment, full-time employees can accrue up to a maximum of 80 hours of PTO and part-time employees can accrue up to a maximum of 16 hours. (See the Accrual Rates Table in employee handbook for additional accumulation breakdowns).

Fitness and Aquatic Center                                                                                                             

State of the Art Fitness and Aquatic Center available to employees free of charge to promote their health and wellness. (No cost to employees if they meet the minimum requirement of 4 visits per month.) Employees may also participate in various fitness activities and challenges throughout the year.

Continuing Education

Lakeview Village's Online University through Care2Learn allows employee to acquire additional skills and knowledge with access to a library of over 500 online courses and Continuing Education Units (CEU's) at no cost.  Additional continuing education may be budged by individual departments.

Education Assistance

It is the policy of Lakeview Village to develop a highly skilled work force by providing educational assistance to its employees. Full-time employees are eligible to apply after one year of full-time service.
